What You’ll Do

• Evaluate patients’ functional abilities in their home environment
• Develop and implement individualized treatment plans
• Help patients improve daily living skills and independence
• Educate patients and families for long-term success
• Collaborate with nurses, PTs, and interdisciplinary team members
• Complete timely and accurate documentation

What You’ll Bring

• Master’s degree in Occupational Therapy
• Active Arizona OT license
• Current CPR/BLS certification
• Home health experience preferred
• Strong communication and organizational skills
• Reliable transportation and valid Arizona driver’s license
