We are currently seeking an experienced, detail-oriented Intake Coordinator to support our Phoenix, Arizona home health operations. As an Intake Coordinator, you will be one of the first points of contact in a patient’s home health journey. Your coordination, attention to detail, and communication skills will help ensure patients receive the care, services, and support they need — when they need it.
You will serve as a key connection between hospitals, physicians, families, insurance providers, and our clinical care team.
What You’ll Do
Manage the intake process from start to finish
Collect and organize patient medical, insurance, and personal information
Coordinate with hospitals, clinics, physicians, and referral sources
Help ensure smooth transitions into home health care
Assess patient needs and match them with appropriate services, treatments, and equipment
Verify insurance coverage and assist with authorizations
Maintain accurate electronic medical records and ensure documentation is complete and compliant
Serve as a compassionate resource for patients and families throughout the intake process
What We’re Looking For
Required: Previous home health experience
Strong understanding of medical terminology and EMR systems
Experience with WellSky/Kinnser preferred
Excellent attention to detail and organizational skills
Strong communication skills with patients, families, providers, and internal teams
Ability to work independently while contributing to a supportive team environment
Flexible, adaptable, and comfortable in a fast-paced setting
Preferred Qualifications
Associate’s or Bachelor’s degree in a healthcare-related field
Experience with insurance verification and authorizations
Bilingual skills are a plus
