Dependable Nursing Inc. is seeking a dedicated Occupational Therapist (OT) to join our growing Home Health team serving the East Valley and greater Phoenix area.

Whether you're looking for full-time or part-time, this position offers flexibility while giving you the opportunity to provide meaningful, one-on-one care in patients' homes.

Your primary territory will be east of I-17. Therapists seeking a full-time caseload should be comfortable occasionally traveling into the West Valley or other areas of greater Phoenix when additional visits are needed to maintain a full schedule.

What You'll Do

  • Evaluate patients' functional abilities and needs in their home environment

  • Develop and implement individualized occupational therapy treatment plans

  • Help patients improve independence with activities of daily living

  • Provide therapeutic interventions focused on safety, mobility, function, and quality of life

  • Educate patients, caregivers, and families on techniques for continued success at home

  • Collaborate with nurses, PTs, physicians, and other members of the interdisciplinary care team

  • Monitor patient progress and adjust treatment plans as appropriate

  • Complete accurate and timely clinical documentation

  • Communicate changes in patient condition to the appropriate clinical team members

What You'll Bring

  • Master's degree in Occupational Therapy

  • Active Arizona Occupational Therapist license

  • Current CPR/BLS certification

  • Home health experience preferred

  • Strong communication, time-management, and organizational skills

  • Ability to work independently while remaining connected to the clinical team

  • Reliable transportation and a valid driver's license

  • Willingness to travel throughout the greater Phoenix area as needed to maintain a full-time caseload
