One-on-One Patient Care

Spend your time helping patients improve their strength, mobility, safety, and independence in the comfort of their own homes.

What You'll Do

  • Perform comprehensive physical therapy evaluations in patients' homes

  • Develop individualized plans of care based on patient needs and goals

  • Provide therapeutic exercises, mobility training, balance training, and other skilled PT interventions

  • Help patients improve strength, function, safety, and independence

  • Educate patients and caregivers on exercises, mobility techniques, and fall prevention

  • Monitor progress and modify treatment plans as appropriate

  • Collaborate with physicians, nurses, therapists, and other members of the interdisciplinary team

  • Complete accurate and timely clinical documentation

What You'll Bring

  • Degree from an accredited Physical Therapy program

  • Active Arizona Physical Therapist license

  • Current CPR/BLS certification

  • Home health experience preferred

  • Strong communication and organizational skills

  • Ability to work independently and manage a field-based schedule

  • Reliable transportation and valid driver's license

  • Flexibility to travel outside your primary territory when needed to maintain a full-time caseload
