What You'll Do

As a Home Health Occupational Therapist, you will work directly with patients in their homes to help them regain independence, improve function, and safely perform the activities that matter most in their everyday lives.

Responsibilities include:

  • Perform comprehensive occupational therapy evaluations in patients' homes

  • Develop individualized plans of care based on each patient's needs and goals

  • Provide skilled occupational therapy interventions to improve strength, mobility, coordination, and functional independence

  • Assist patients with activities of daily living and strategies for safely completing everyday tasks

  • Evaluate the home environment and recommend modifications or adaptive equipment when appropriate

  • Educate patients, families, and caregivers on techniques to promote safety and independence

  • Monitor patient progress and adjust treatment plans as necessary

  • Complete accurate and timely clinical documentation

  • Communicate changes in patient condition to physicians and members of the interdisciplinary care team

  • Coordinate care with nurses, physical therapists, speech therapists, and other healthcare professionals

  • Maintain compliance with agency policies and applicable state and federal regulations

Service Area

This position will primarily cover territories east of I-17, including:

  • Central Phoenix

  • Tempe

  • Scottsdale

  • Paradise Valley

  • Surrounding communities within the assigned territory

What We're Looking For

  • Current Arizona Occupational Therapist license in good standing

  • Graduate of an accredited Occupational Therapy program

  • Current CPR certification

  • Valid driver's license and reliable transportation

  • Strong clinical assessment and documentation skills

  • Excellent communication and time-management abilities

  • Ability to work independently in a home-based environment

  • Home health experience preferred, but qualified OTs with other clinical backgrounds are encouraged to apply
