Dependable Nursing is seeking a compassionate and experienced PRN Medical Social Worker (MSW) to join our growing Home Health team in the Phoenix area.

We have opportunities available in the East Valley, West Valley, or both, depending on your location and availability!

What You'll Do

• Conduct psychosocial assessments for home health patients

• Identify emotional, social, financial, and environmental factors affecting patient care

• Develop individualized social work care plans in collaboration with the interdisciplinary team

• Provide counseling, emotional support, and education to patients and families

• Connect patients with community resources, financial assistance, and support services

• Assist patients and families with long-term care planning and healthcare transitions

• Advocate for patient needs and promote independence and quality of life

• Coordinate with nurses, therapists, physicians, and other healthcare professionals

• Maintain accurate, timely documentation in the electronic medical record (EMR)

• Ensure compliance with agency policies and applicable home health regulations

What We're Looking For

• Master's degree in Social Work (MSW) preferred

• Bachelor's degree in Social Work may be considered if applicable home health qualification requirements are met

• Current Arizona social work license, as applicable

• Minimum one year of supervised social work experience in a healthcare setting preferred

• Previous Home Health experience strongly preferred

• Knowledge of community resources and healthcare support programs

• Strong communication, assessment, and problem-solving skills

• Compassionate, dependable, and comfortable working independently

• Experience with electronic medical records (EMR)

• Valid driver's license, reliable transportation, and current auto insurance
