As a Medical Social Worker in this position, you will be responsible for implementing the patient's plan of care, assessing and providing patient reports and assisting family members with patient self-care, and coordinating home health services.

What we are looking for:

  • Must hold, in good standing, a license as a Medical Social Worker issued by the state in Arizona
  • Master’s Degree
  • A minimum of one (1) full year of supervised post-certification experience in a healthcare setting.
  • Home health experience preferred.
