What You’ll Do

• Perform patient assessments and develop individualized care plans

• Educate patients and families on treatment, medication, and follow-up care

• Collaborate with care coordinators and other clinicians for seamless service

• Complete timely documentation using OASIS standards

What You Bring

• Active Arizona RN license

• Home health experience, including OASIS documentation

• CPR/BLS certification (or willingness to obtain)

• Valid AZ driver’s license & reliable transportation

• TB test & fingerprint clearance (we can help with both)

• Bilingual (Spanish) is a plus

• Graduate of an accredited RN program
