What You’ll Do

• Drive Growth: Use your established relationships to generate referrals and boost patient admissions.

• Build Bridges: Cultivate lasting partnerships with physicians, hospitals, skilled nursing facilities, and referral sources.

• Lead Strategy: Design and execute a market strategy that hits — and exceeds — growth goals.

• Spread the Word: Promote our services through compelling presentations, networking, and outreach events.

• Support Partners: Be the go-to resource for referral sources, ensuring satisfaction and retention.

• Collaborate for Care: Work alongside clinical teams to deliver seamless, high-impact care transitions.

What We’re Looking For

• Proven sales success in home health or hospice experience is a MUST

• Strong existing book of business and referral relationships in the Phoenix market

• Confident communicator and strategic thinker

• Organized, goal-driven, and self-motivated

• Familiarity with Medicare/Medicaid a plus
