What You’ll Do

• Conduct in-home assessments and develop personalized care plans

• Educate patients and families on medications, conditions, and follow-up care

• Document visits and patient progress with accuracy using OASIS

• Coordinate with physicians and the care team to ensure seamless service

• Build trust and connection as a consistent, compassionate presence in your patients’ lives

What You Bring

• Active Arizona RN license and current CPR/BLS certification

• Home health experience and strong OASIS documentation skills

• Reliable transportation, valid driver’s license, and clean driving record

• TB test and AZ fingerprint clearance (we’ll assist with this if needed)

• Bilingual skills—especially Spanish—are a plus

• RN degree from an accredited program
