What You’ll Do

Create Personalized Plans

• Assess mobility and functional limitations

• Develop evidence-based, individualized therapy plans

• Apply proven techniques to restore movement and improve quality of life

• Track and adjust treatments to maximize outcomes

Empower Patients

• Educate and motivate patients and families throughout their recovery

• Promote independence, confidence, and sustainable progress

• Celebrate every milestone—big or small

Collaborate Across Care Teams

• Coordinate with RNs, OTs, physicians, and caregivers

• Maintain clear communication and accurate documentation for seamless care delivery

What You Bring

• Active Arizona Physical Therapist license

• Minimum 1 year of home health experience preferred

• Strong clinical judgment, organization, and communication skills

• Genuine passion for patient-centered, in-home care

• Reliable transportation and a drive to make a difference
