**Job Summary:**
As a Home Health RN or LPN, you will be responsible for delivering high-quality nursing care to patients in their homes. You will conduct assessments, administer medications, and collaborate with healthcare teams to develop and implement care plans tailored to each patient's needs. Your role will be vital in promoting independence and improving the quality of life for our patients.

**Key Responsibilities:**
- Conduct comprehensive patient assessments to determine health status and needs.
- Develop and implement individualized care plans in collaboration with the healthcare team.
- Administer medications and treatments as prescribed by physicians.
- Monitor patient progress and make necessary adjustments to care plans.
- Educate patients and their families on health management, medications, and treatment plans.
- Maintain accurate and timely documentation of patient care and progress.
- Perform wound care, catheter care, and other nursing procedures as required.
- Collaborate with interdisciplinary teams to coordinate patient care effectively.
- Provide emotional support and guidance to patients and their families.

**Qualifications:**
- Current and valid RN or LPN license in Arizona.
- Previous home health experience preferred but not required.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Compassionate and patient-centered approach to care.
- Current CPR/BLS certification.
