Licensed Practical Nurse (LPN) – Home Visits

This is your chance to provide high-quality, personalized care—right where patients feel most comfortable: their own homes.

What You’ll Do:

• Visit patients in their homes to assess health needs, monitor progress, and provide skilled nursing care.

• Administer medications and treatments with precision and compassion.

• Support patients with daily living activities to help them stay independent.

• Partner with RNs and other healthcare professionals to create and carry out individualized care plans.

• Educate patients and families on health management, medications, and wellness.

• Maintain accurate and timely documentation in electronic health records.

• Offer emotional support and encouragement every step of the way.

What You Bring:

• Active LPN license in the state of Arizona.

• At least 1 year of nursing experience (home health experience a plus).

• Strong assessment, critical thinking, and communication skills.

• Self-motivated, organized, and compassionate.

• Current CPR certification.

• Reliable transportation and willingness to travel across Tucson.
