What You’ll Do

• Assess each patient’s physical, cognitive, and emotional abilities.
• Create and carry out individualized care plans that empower independence in daily living.
• Educate patients and families to promote long-term success at home.
• Partner with nurses, PTs, and other healthcare professionals for coordinated, holistic care.
• Maintain accurate documentation and track progress.

What You’ll Bring

• Education: Master’s in Occupational Therapy from an accredited program.
• License: Active Arizona OT license.
• Certification: Current CPR/BLS.
• Experience: Prior home health experience preferred.
• Skills: Excellent communication, organization, and time management.
• Transportation: Reliable vehicle and valid Arizona driver’s license.
