Position Summary
We’re seeking a dedicated Registered Nurse (RN) Case Manager in the Kingman area to oversee and coordinate in-home nursing services for DDD members. This role is primarily remote, with home visits to members, monthly office visits to our Phoenix location, and participation in an on-call rotation for urgent care needs.
You’ll manage care plans, support field nurses, and ensure that each child receives compassionate, high-quality care that aligns with DDD and DNI standards.
Key Responsibilities
- Conduct home visits to assess member health and care needs.
- Develop, implement, and update individualized care plans.
- Provide clinical training, mentorship, and support to field nurses.
- Collaborate closely with families, support coordinators, and healthcare providers.
- Respond to urgent issues and participate in on-call rotation.
- Attend monthly meetings at the Phoenix office for training, collaboration, and case review.
- Maintain accurate and compliant documentation per DDD and DNI policies.
Qualifications
- Active Registered Nurse (RN) license in the State of Arizona (required).
- Minimum 2 years of pediatric or home health experience preferred.
- Strong communication, leadership, and organizational skills.
- Comfortable traveling locally for home visits and monthly to Phoenix.
- Reliable transportation and valid driver’s license required.
