Qualifications

• Active LPN License: Current and valid with the Arizona State Board of Nursing.

• Home Health Experience: Required; WellSky (Kinnser) experience preferred.

• Driver’s License: Valid Arizona driver’s license required.

• Reliable Transportation: Must have dependable vehicle and auto insurance.

• CPR Certification: Current and valid.

• TB Clearance: Current health card or ability to obtain one per federal and state guidelines.

• Education: Graduate of an accredited Licensed Practical Nurse (LPN) program.
