RN/LPN Essential Job Functions

• Provide in-home nursing care for low to high acuity patients

• Complete patient assessments and follow individualized plans of care

• Administer treatments and medications as ordered

• Collaborate with families and interdisciplinary team members to achieve positive patient outcomes

• Document accurately and communicate patient changes to the RN Clinical Supervisor/Site Director

• Safely lift, position, or transfer patients, supplies, and equipment

Qualifications

• Active Arizona RN or LPN License (or Compact License)

• Valid Arizona Class D Driver’s License

• Current BLS Certification (online courses not accepted)

• Current TB test or Chest X-ray within the last 12 months

• Level One Fingerprint Clearance Card

• Article 9 Training

• Strong time management, communication, and problem-solving skills
