Your Responsibilities:
Patient Care
Conduct assessments of patients’ functional, emotional, and physical capabilities
Develop and implement customized care plans focused on daily living skills and independence
Provide hands-on therapy in the home environment
Education & Collaboration
Teach patients and families adaptive techniques and safe home practices
Work closely with nurses, PTs, and the broader care team to ensure coordinated treatment
Maintain accurate, timely documentation and progress notes
Continued Development
Stay current with best practices in occupational therapy and home health care
What We’re Looking For:
Licensed OT – Degree in Occupational Therapy and active Arizona license
Experienced Clinician – Previous home health experience is required
CPR/BLS Certified
Reliable & Mobile – Valid AZ driver’s license and dependable transportation
Strong Communicator – Excellent interpersonal, time management, and documentation skills
