Your Responsibilities:

Patient Care

  • Conduct assessments of patients’ functional, emotional, and physical capabilities

  • Develop and implement customized care plans focused on daily living skills and independence

  • Provide hands-on therapy in the home environment

Education & Collaboration

  • Teach patients and families adaptive techniques and safe home practices

  • Work closely with nurses, PTs, and the broader care team to ensure coordinated treatment

  • Maintain accurate, timely documentation and progress notes

Continued Development

  • Stay current with best practices in occupational therapy and home health care

What We’re Looking For:

  • Licensed OT – Degree in Occupational Therapy and active Arizona license

  • Experienced Clinician – Previous home health experience is required

  • CPR/BLS Certified

  • Reliable & Mobile – Valid AZ driver’s license and dependable transportation

  • Strong Communicator – Excellent interpersonal, time management, and documentation skills
